Forms for Injured WorkersYou can forward any off-work slips, wage information, medical bills, notes, and other relevant documents to [email protected]. This email is monitored throughout each business day, ensuring that all submitted documents are promptly uploaded into TRISTAR's claims system for review and action by the assigned Claims Examiner.
